Present a completed Disabled Persons Archery Hunting Permit [PDF, 69 KB] form signed by a physician at any IDFG Regional office. Physician certifies that applicant has a permanent disability whereby they do not have use of one (1) or both of his arms or hands. Available ONLY at Fish and Game Offices
 Disabled Persons - Motor Vehicle 
 $ 1.75 
Present a completed Disabled Persons Motor Vehicle Hunting Permit [PDF, 290 KB] form signed by a physician at any IDFG Regional office. Physician certifies the applicant is disabled and has severe mobility impairment per Idaho Code. The words "permanently disabled" on reverse side of Idaho Drivers license is acceptable in lieu of physician certification form.  Fishing - State Long-term Care Facility 
 $ 1.75 
Submit application form [PDF, 194 KB] at any IDFG Regional/HQ Office. This permit is only available to State Hospital North, State Hospital South, Idaho State School and Hospital, and State Veterans Homes.
 Fishing - Two-pole

 Hound Hunter

 Migratory Bird

Federal Migratory Game Bird Harvest Information Program Validation. Dove, duck, goose, coot, sandhill crane and common snipe.
